Assessment Details: I have successfully conducted a physical examination of Bubba McFord which was limited to a questionnaire pertaining to the subject's medical history and then followed by a brief inspection utilizing a stethoscope and a sphygmomanometer.
Conclusion: Both BPM and BP were recorded at average levels with his BP having been confirmed at a solid 120/80, and his heart-rate resting at about seventy beats per minute. I hereby clear Mr. Bubba McFord as physically fit for duty.
