Assessment Details: I have successfully conducted a physical examination of Michael Werenston which was limited to a questionnaire pertaining to the subject's medical history and then followed by a brief inspection utilizing a stethoscope and a sphygmomanometer.
Conclusion: Both BPM and BP were above the average levels with his BP having been confirmed at a solid 130/80, I have made sure to give him the proper remedies, due to his suspension being lifted I have decided to refer him to the Central Desert Behavioral Health Hospital in Albuquerque, New Mexico. The details of this prior incident have been classified I believe, and thus I will exclude it from the report.
Human Resources has been notified of this.
