Good Day Everyone,
This is a shared message sent to all staff registered to the intranet, attached to this email is a medical record form that needs to be filled out in order to meet our health and safety guidelines. In addition to this, it's imperative the medical team knows of any allergies to medication you may have (such as penicillin) in the event we need to use it.
The form should take you around five minutes to fill it, once completed please scan it and reply to this email so I can forward it to our database.
Addendum: Due to having been asked this question a few times, I'll state it here on the email. You do not need to fill in the Advanced Care Plan (ACP), but we would like you to declare your stance on CPR found on page two "DNACPR FORM", you also don't need complete the last page, that is for healthcare professionals to complete on any admissions to the clinic you have.
